How To Use The PHQ Report CardStep One: Select Your Search Criteria On the main search page, check the Terms and Conditions box and click Search to view a list of all certified organizations. To view results for a specific organization, enter the name of the organization in the search box. Check the box indicating that you agree to the Terms and Conditions and click Search. Step Two: View PHQ Organizations The Search Results page lists all organizations that meet your search criteria by organization name, organization type, evaluation type and current organization status. Step Three: View Detailed Organization Information and Physician Quality Results To view additional information about organizations that have been evaluated on the physician quality standard, click the name of the organization on the Search Results page. For organizations that have completed an evaluation under the Physician Quality standards portion of the Physician Hospital Quality evaluation, or the PHQ 1 standard, this page will include detailed information about their scores and results on the factors and elements.
